Question types you can use

The builder's list on the left has two groups: Form fields (questions people answer) and Design elements (headings, text and layout). Drag any of them onto your form.

Form fields

Short answer: one line of text, like a name.
Long answer: a bigger box for details.
Email: an email address.
Phone: a phone number. It accepts numbers with spaces, +, brackets and dashes.
Number: numbers only.
Calculation: works out a total from other answers. See Calculations.
Date: a date picker, shown as dd/mm/yyyy. Under More options you can tick Ask for a time as well or Fill in today's date automatically.
Booking: people pick a day and time. See Bookings and time slots.
Dropdown, Choose one, Choose several: lists of options. See Lists of options.
Tick box: one box to tick, like "I agree".
Yes / No: a Yes and No choice side by side.
Address: street, suburb, state, postcode and country. It can suggest Australian addresses as people type.
Country: a list of countries, with Australia first.
File upload: people attach a file, such as a licence or certificate.
Signature: people sign with a finger or mouse.
Choice matrix: several questions with the same answers in columns, like a rating table.
Payment: takes a card payment. See Take card payments in a form.
Check work rights: checks a visa with Home Affairs. See Add a work rights check to a form.

Design elements

Heading: a title inside your form.
Section bar: a coloured bar with a section title.
Formatted text: instructions or notes, with bold, sizes, colours and lists.
Image or logo: a picture, in small, medium, large or full width.
Divider: a line across the form.
Box / page: groups questions under a title. See Pages and sections.
Repeat group: a set of questions people can fill in more than once, with an Add another button. Handy for work history or referees.

Good to know

Every question can be made Mandatory, given help text, and shown or hidden by an earlier answer. See Add and edit questions and Show or hide questions.
For File upload, More options lets you allow several files, choose the allowed file types and set the largest file size.
