Add your letterhead

Your letterhead makes your forms and their PDFs look like your own paperwork. You set it up once for each company, and every form of that company shows it at the top and bottom, and on every page of the response PDFs.

Set it up from a PDF

  1. On your dashboard, click Edit company on the company's card.
  2. Under Form letterhead, at Cut it from a letterhead PDF, choose your letterhead PDF.
  3. Formhub shows the first page with two lines. Drag Header ends to the bottom of your header, and Footer starts to the top of your footer.
  4. To leave out the header or the footer, drag its line to the very top or bottom of the page.
  5. Click Save.

Letterhead in Word? In Word, choose File, Save As, PDF, then pick that PDF.

Or upload pictures

Under Header picture and Footer picture, choose a PNG or JPG for each. A wide picture fills the width of the form. Then click Save.

Turn it off for one form

  1. Open the form in the builder and click Details at the top.
  2. Untick Use the company letterhead (header and footer).
  3. Click Save.

Good to know

  • When the letterhead is on, the company logo isn't shown again at the top, because the letterhead usually has it already.
  • Show the header above the form, also under Details, controls the bar with the logo and form name. Turn it off if your letterhead or the form already has its own heading.
  • To remove the letterhead, tick Remove header or Remove footer on the company page, then click Save.
  • The PDF can be up to 20 MB.
  • The builder shows the letterhead above and below your questions, so you can see how it looks. Check it on a phone too with Phone view.
  • New forms use the letterhead straight away. See also Add a company for the logo and colours.
